Individual

The cost of membership for an individual is:

$77 per month, or

$231 per quarter, or

$462 every 6 months, or

$847 per year

For new patients, there is a one-time enrollment fee of $77.

Couple

$137 per month, or

$411 per quarter, or

$822 every 6 months, or

$1507 per year

For new couples, there is a one-time enrollment fee of $137.

Family

$198 per month up to 4 people. Each additional child, add $10

$594 per quarter up to 4 people. Each additional child, add $30

$1188 every 6 months up to 4 people. Each additional child, add $60

$2178 per year up to 4 people. Each additional child, add $110

For new families, there is a one-time enrollment fee of $198.

(A family is defined as parent(s) and unmarried children under the age of 26 residing in the same household.)
