The cost of membership for an individual is:

$72 per month, or

$216 per quarter, or

$432 every 6 months, or

$792 per year

For new patients, there is a one-time enrollment fee of $72.

Couple

$129 per month, or

$387 per quarter, or

$774 every 6 months, or

$1419 per year

For new couples, there is a one-time enrollment fee of $129.

Family

$186 per month up to 4 people. Each additional child, add $10

$558 per quarter up to 4 people. Each additional child, add $30

$1116 every 6 months up to 4 people. Each additional child, add $60

$2046 per year up to 4 people. Each additional child, add $110

For new families, there is a one-time enrollment fee of $186.

(A family is defined as parent(s) and unmarried children under the age of 26 residing in the same household.)
